Puerto Rico is United States territory, so the aviation framework is the American one. Around San Juan the airport keeps the airspace tight, and the old city wall seen from the water is the one shot worth the paperwork that goes with it.
The east coast is a different matter. Around Fajardo and out towards Culebra and Vieques there is little restricting airspace and far fewer boats than in the Virgin Islands proper, which makes it the practical area for charter and marina work.
